Overview Purpose: To improve the provision of comprehensive disease prevention, health promotion, and primary care services to Native Hawaiians • Approximately $12.2 million available • Six grants • Project period start date: August 1, 2013 • Project period: One (1) year • Grants.gov due date: June 7, 2013 at 11:59 PM ET

Overview of Changes • Central Contractor Registration (CCR) transitioned to the System for Award Management (SAM) on July 30, 2012 • All sample documents, forms, and templates are available at the technical assistance web site http://www.hrsa.gov/grants/apply/assistance/NHHCS • 3 additional Performance Measure options have been added: • Coronary Artery Disease (CAD): Lipid Therapy • Ischemic Vascular Disease (IVD): Aspirin Therapy • Colorectal Cancer Screening

Project Narrative & Review Criteria Project Narrative provides a comprehensive description of all aspects of the proposed project Review Criteria are utilized by reviewers to assess the application: • Need (10 points) • Project Update (15 points) • Response (35 points) • Resources/Capabilities (10 points) • Evaluative Measures (10 points) • Impact (10 points) • Support Requested (10 points)

  9. Performance Measures • Applicants must include at least 6 of the 14 sample core Clinical Performance Measures • 6 additional sample areas for inclusion of additional performance measure goals: • Behavioral Health • Oral Health • Hearing/Otitis Media • Traditional Healing • Health Education and Disease Prevention • Nutrition and Physical Activity • Applicants must include at least 2 of the 6 sample core Financial Performance Measures

Clinical Performance Measures • Diabetes • Cardiovascular Disease • Cancer • Prenatal Health • Perinatal Health • Child Health • Weight Assessment and Counseling for Children and Adolescents • Adult Weight Screening and Follow-Up • Tobacco Use Assessment and Counseling (Tobacco Use Assessment) • Tobacco Use Assessment and Counseling (Tobacco Cessation Counseling) • Asthma – Pharmacological Therapy • Coronary Artery Disease (CAD): Lipid Therapy • Ischemic Vascular Disease (IVD): Aspirin Therapy • Colorectal Cancer Screening

  12. Financial Performance Measures • Total cost per patient • Medical Cost per Medical Visit • Change in Net Assets to Expense Ratio • Working Capital to Monthly Expense Ratio • Long Term Debt to Equity Ratio • Non-Federal Matching Funds (percentage of matching funds included in the total project budget)
