  3. Letsels zichtbaar onder echografie : corebiopsie • Letsels enkel zichtbaar op mammografie, geen echografisch correlaat : • Microverkalkingen • Opaciteit • distorsie

  4. Vroeger : excisiebiopsie of controle mammografie met verdere follow-up • Nu : (meestal) mammotoombiopsie: indicaties: • Verdachte of twijfelachtige microverkalkingen, microverkalkingen die veranderd zijn • Verdachte opaciteit of distorsie zonder echografisch correlaat

  5. techniek • Biopsie onder mammografische geleide • Zijlig of zittend • In ons ziekenhuis : mammotoomtafel : buiklig • Patient is meer relaxed • Minder kans op beweging van patient • Zwaartekracht : prepectorale regio

  9. Steeds profielopname nodig • Goede positionering • Uiteraard locale verdoving • Postpunctiehematoom : langdurig afdrukken • Uitslag na ongeveer 4 werkdagen

  10. resultaten • DCIS, LCIS • Apocrienemetaplasie, atypische ductaalepitheelhyperplasie • Dysplasie, benignemicroverkalkingen • Onze resultaten 33% maligne • relatief veel echografische geleide corebiopsies • Anderzijds : zekerheidsdiagnose met minimaal invasieve procedure, op ambulante basis • Geen twijfel, geen follow up

  11. Take home message : • Mammotoombiopsie als oplossing voor letsels die niet zichtbaar zijn op echografie. • Dossierkennis op voorhand, met profielopname en vorige opnamen • Zekerheidsdiagnose op minimaal invasieve wijze.
