This study presents the outcomes of the BIG (Brain Injury Guidelines) project, which aims to define effective management protocols for traumatic brain injury (TBI) by acute care surgeons. Conducted from 2009 to 2011, the retrospective analysis involved a cohort of 3,803 patients, focusing on non-surgical management, trauma kinematics, physical examinations, and imaging studies. Key findings include the need for initial management in level 1 institutions and the appropriate indications for CT imaging and neurosurgical evaluation, paving the way for standardized TBI care.
