IVT Request Form and Dashboard • New StarPanel IV Therapy (IVT) Request Form for IVT bedside assistance • Will Improve efficiency for both nursing and IVT • No waiting for IVT to come to phone or call back • From the Dashboard IVT has the information needed to correctly prioritize their work WHEN: September 23rd WHO and HOW: • RN’s or MR’s will complete the IVT Request Form instead of calling or paging IVT – information will flow to IVT Dashboard • IVT will monitor the IVT dashboard and plan their work by priority • IVT Dashboard will display all information entered in the form

IVT Request Form • IVT Hours on Duty clearly display Things to Consider: Only fill out the sections that are pertinent to the patient’s situation The user must complete the RN Requesting Service field – enter the name and contact number of the RN IVT should follow up with. • There must be an active order in WIZ/HEO for IVT to access a Port-a-cath • The assessment/Eval section is to request the IVT tech come to the patient bedside to assess the usefulness of the IV or PICC line • Be sure to list any pertinent details in Comments Must complete Must complete

IVT Dashboard • For nurses to track their IVT request, completed forms display on the IVT Dashboard -IVT will remove when completed -The nurse will remove if IVT services no longer needed -IVT team will assign a High, Med, or Low priority -IVT will enter initials to Priority box to identify which IV therapist is assigned to the request Specific info taken from request form for IVT team to prioritize requests JH High Note column: For both the nurse and IVT tech to communicate. To keep nurses updated, IVT will acknowledge they are aware of the request = Not acknowledged = Not acknowledged after 1 hour = Acknowledged Link to original document

IVT Dashboard: Set as a Favorite • Save as a Favorite in Menu Bar • Configure your screen to view the VUH or VCH Dashboard Save IVT Dashboard to Favorites Click on Dashboards, to expand dashboards list, click Ancillary Dashboards Left click and hold on IVT, drag up to favorites until favorites section turns green Release left click a c b Configure Click change panel Choose VUH or VCH a b One time set-up!!

IVT Coverage • VUH (adult hospital): IVT normal coverage hours on duty: • Mon-Wed 0700-1900, 2100-0700 • Thurs-Sunday 07-1900 • VCH: 24 x 7 coverage scheduled • There is no on-call or back-up coverage for IVT • 3 ways to determine if IVT is on duty • IVT staffing hours will be communicated to the AOC , passed along to CSL at bed meeting • A message will continue to be placed on the IVT pager • Refer to the request form for the most up to date IVT hours on duty

Please Consider… • RN should submit this form to request IVT when in house- no need to text IVT or submit requests for IVT assistance through the MR • IF MR does get the paper IVT request from a nurse she should submit request via this form if they are in house • RN can enter a note in the dashboard to communicate to IVT if pt has to leave the floor after a request is entered before completed. • RN should partner with the IVT by removing entries from IVT Dashboard that are no longer needed • System Support Staff will provide 24/7 onsite support in both VUH & VCH. • If you have questions or need assistance: • Call the Help Desk at 343-4357 (3-HELP)
