Department of Medical Assistance Services The Work You Do Is So Critical! • Transition Coordination/Case Management • Meet the person where they are (Person in Environment) • Empower the individual to be a part of their re-integration into the community • Partner with individual to address self-identified and assessed biopsychosocial and spiritual needs • Locating resources • Connecting to services • Referrals to community based agencies http://dmasva.dmas.virginia.gov 2

Strategies for Success • Research • Try doing your own, mini “community assessment” • What are the different types of community-based entities in my service area(s)? • Governmental • State, Federal (including military bases) • Nonprofit • For profit • Corporate or small business • Civic
Strategies for Success continued… • Network • Don’t discredit collaborating with community based entities that may not seem to be related to older adults or individuals with disabilities. • Any agency that deals with or provides human services will more than likely be a good resource, now and in the future. • Such agencies may possess or know of resources that can be beneficial for MFP participants, and you may know of resources to share with them in return.
