Department of Medical Assistance Services The Work You Do Is So Critical! • Transition Coordination/Case Management • Meet the person where they are (Person in Environment) • Empower the individual to be a part of their re-integration into the community • Partner with individual to address self-identified and assessed biopsychosocial and spiritual needs • Locating resources • Connecting to services • Referrals to community based agencies http://dmasva.dmas.virginia.gov 2

  4. Strategies for Success • Research • Try doing your own, mini “community assessment” • What are the different types of community-based entities in my service area(s)? • Governmental • State, Federal (including military bases) • Nonprofit • For profit • Corporate or small business • Civic

Strategies for Success continued… • Network • Don’t discredit collaborating with community based entities that may not seem to be related to older adults or individuals with disabilities. • Any agency that deals with or provides human services will more than likely be a good resource, now and in the future. • Such agencies may possess or know of resources that can be beneficial for MFP participants, and you may know of resources to share with them in return.

Types and Examples of Agencies • Local Financial Aid/Resource Referrals: • Statewide: The Salvation Army • http://virginiasalvationarmy.org/locations/ • Services offered include rapid re-housing assistance, employment counseling, classes, case management, rental/mortgage/utility assistance, thrift stores. Area commands located throughout the state of Virginia. • Hampton Roads area: Helper’s Organization • http://www.thehelperswillhelp.org/ • Offers free local resource referrals and financial aid on a case by case basis individuals and families in the Hampton Roads area.
