  4. 背景說明 • 全文為：Clinical Document Architecture • 發行版本： • ANSI/HL7 CDA R1.0-2000 • ANSI/HL7 CDA R2.0-2005 • 已是ISO標準(TC 215 / ISO/HL7 27932) • 主要維護者是：HL7 Structured Documents Technical Committee (SDTC) • 主要是定義資料交換之規範，使用到的技術有： • XML, • HL7 Reference Information Model (RIM) • Version 3 methodology • Vocabulary (LOINC,SNOMED, ICD, local等)

  5. CDA是電子文件交換規範 • CDA 可以是 • 出院病摘 • 護理記錄 • 病程記錄 • 檢驗報告 • 健康報告 • 可以組合任何已簽章資料的內容

  6. 甚麼是電子文件 • 從XML角度，任何一種都是“document”。 • Documents從醫療的角度： • 表現出歷史醫療記錄格式。 • 把分散的資料及片斷病歷敍述整合在一起。 • CDA的目的就是將健康照護過程中所產生的電子文件進行規範。

  7. 定義CDA特性 • Persistence：可作為永久儲存之格式 • 採XML技術可永久儲存。 • Stewardship：可供文件管理系統管理 • 內含版本控管機制與其他文件資訊，可供DMS來使用。 • Potential for authentication：可具備鑑別性 • 因是電子文件可被簽章。 • Context：文件具有脈絡性 • 透過CDA的結構描述，可將文字敘述中的前後文關係表達出來。 • Wholeness：欄位具備完整性 • 透過CDA R2的結構可充分表達病歷文件之資訊。 • Human readability：資訊可由人直接讀取 • 透過瀏覽設計可得到與紙本病歷相同樣式，滿足閱讀習慣。

  10. cda與HL7訊息的關係 • HL7 訊息是交換CDA文檔的補足說明資料。 • CDA是完整定義的資訊物件，是獨立於訊息之外。 • CDA文件可用MIME編碼放入HL7 訊息中。

  21. CDA 檔頭 • 檔頭描述：(Level 1) • 文件本身(unique ID, document type classification, version) • 參與者 (providers, authors, patients…) • 文件關係者 (to orders, other documents…) • 這些中介資料是要給文件管理系統使用的。

  23. XML 本文 • 有兩大類型描述區塊 • 人類可讀（Human-Readable） • 敘述區塊（narrative block）用來表達與臨床、法律所需的內容。 • 一般而言，仍須透過xslt技術將其轉換成具有呈現格式的文件，如html或pdf。 • 定義上屬於Level 2。 • 機器可讀（Machine-Readable）─ Optional • 屬CDA條目的部份，都是系統自動產生。 • 系統可透過CDA R2的結構關係，搜尋特定編碼，取得資訊。 • 定義上屬於Level 3。

  45. 瞭解CDA R2版本控管方式 子文件 父文件 id：文件流水號，只要新產生文件，就給予一個流水號。 setId：文件序列號，第一份文件賦予新序列號。 append時：新序列號。 replace時：同序列號。 versionNumber：版本序，辨識同一文件序列號者。(replace才會新增序號)
