Please ensure you select your correct year group when you log on • At the bottom of each page there is a forward arrow you will need to press to go on through the survey • For some of the longer pages you will need to scroll down • The data is collected from each page when you press the forward button (if you don’t click on the slider and if it doesn’t change colour then the answer hasn’t been recorded)

  3. Please check you are happy with the your answers before you press the forward button – once you have done this you cannot go back. Please think carefully before you answer. If you do not understand a question – please ask before you select your answer.

  4. The survey is anonymous • There are some questions about where you live and your home situation For example which council district you live in e.g. Forest of Dean, Gloucester, Stroud, Cotswolds, Cheltenham, or Tewkesbury • You will also be asked your home post code – just the 1st four digits (this will only identify the general area you live not your street)

  5. Your answers are anonymous! This means you cannot be identified. • Your answers are extremely important • They will be used to help the school understand more about you • It’s very important you are honest and sensible

  6. Healthy Eating/ Physical Activity • Some questions ask you things such as ‘how many portions of fruit and vegetables do you generally eat in a day?’ To answer this you will need to give an average. Ask if you’re not sure! • For the physical activity question, the definition that is used is an activity that ‘makes you breathe harder and faster than you normally would ‘

  7. Alcohol For the question ‘Do you drink alcohol?’ • This refers to a proper alcoholic drink - a whole drink, not just a sip (Please don't count drinks labelled low alcohol).

  8. There is a question about whether you need more knowledge about keeping safe from grooming. “Grooming is when an older person starts a relationship with a child in order to sexually abuse that child” The question which asks how many days did you miss school last term – does not include days out on school activities such as trips. Remember this is important be honest and sensible –and ask if you’re aren’t sure!!!

  9. Bullying For the question ‘Have you been seriously bullied in the last year? Remember: bullying is something that is repeated , it’s not a disagreement or a one off fight or argument)
