INSTRUCTIONS (Please read carefully): You will need to modify this template to provide CADCA with one Big Picture Overview slide for each substance use/abuse problem you are highlighting in your application. We have provided these examples to give you a head start on formatting. Each “problem” will be in a RED box. Please assign each of your identified root causes with their own colored box, and assign the local conditions associated with each root cause in their own colored circle (Don’t worry if you make a mistake with the colors! It’s just a way to use color to make the applications easier to review and to demonstrate the linkages between root causes and local conditions.) Feel free to move the shapes around as you please and please fit the whole “big picture” for each substance use/abuse problem on one slide. We are looking for basic statements (a brief sentence) to describe each of your problems, root causes, and local conditions. Please see the example on page 9 of your application instructions. Please do not include data on this template. This will serve as a cover page for the data and data sources you provide to back up the coalition’s problem analysis in your “Data/Interventions” templates.
