  5. Possible causes of reaction • Q4 • Select in your work book the reasons a transfusion reaction may have occurred i.e. what could have gone wrong?

  6. In Groups • Discuss how the following scenarios may occur • Wrong blood in tube (WBIT) • Right blood to wrong patient • Transfusion associated circulatory overload

  7. Identify the errors on yellow form. Give possible reasons for them.
