Should questions arise at time of dispensing related to appropriateness of the order, and the patient requires counseling, the member must ensure that the patient has access to a licensed pharmacist that can deliver counseling in a manner appropriate to the patients’ circumstances. Safety regulation #12 Safety regulation #12 The roles of the member and the dispensing partners must be clearly de?ned in writing to cover all requirements of the CIPA standards and be included in the member’s standard operating procedures. Safety regulation #14 Safety regulation #14 Active members shall maintain and enforce policies and procedures that assure the integrity, legitimacy, and authenticity of the Prescription Drug Order and seek to prevent Prescription Drug Orders from being submitted, honored, and ?lled by multiple pharmacies. Active members shall maintain and enforce policies and procedures that assure that prescription medications are not prescribed or dispensed based upon telephonic, electronic, or online medical consultations without there being a pre-existing patient-prescriber relationship that has included an in-person physical examination. Safety regulation #15 Safety regulation #15 Active members shall request full disclosure of, and maintain a current list of, all medications and devices forming part of the patients’ therapy. Active members shall maintain policies and procedures to ensure that routine monitoring of the patient’s medications for potentially harmful drug interactions takes place. Safety regulation #16 Safety regulation #16 In jurisdictions where it is necessary for the original prescription to be converted into a domestic prescription for ful?llment by the dispensing pharmacy, the member shall ensure that the secondary reviewing physician assessing the patient’s personal health information conducts an independent review to determine whether a domestic prescription should be issued. Safety regulation #17 Safety regulation #17 Active members shall maintain appropriate policies and procedures with respect to generic substitution of medications. Active members shall ensure that patients are told if a generic substitution is available. Patients may choose the generic, but if the physician has indicated no substitution, then the physician must be contacted for approval prior to dispensing a generic. If refused by the physician, the pharmacy shall notify the patient that the brand name will be dispensed. Safety regulation #18 Safety regulation #18 Patients must receive medication that will not expire during the course of therapy as indicated on the prescription. Page 3

For greater certainty, no member shall sell or dispense controlled substances or narcotics into the United States. Safety regulation #21 Safety regulation #21 Active members shall maintain and enforce policies and procedures which are in accordance with the statutes and regulations within the jurisdiction where licensed regarding the protection of the personal health information of patients, and shall ensure that there are appropriate safeguards in place to prevent inappropriate or non-essential access or use of a patient’s personal health information. Safety regulation #22 Safety regulation #22 Active members shall ensure that the protection of con?dentiality is an integral part of sta? training and that a written agreement with each sta? member is in place to enforce the protection of patient information. Safety regulation #23 Safety regulation #23 Active members shall use patients’ speci?c health information only to provide pharmaceutical services. Patient health information may be shared with other healthcare providers to enhance patient care and as agreed by the patient. Safety regulation #31 Safety regulation #31 Active members shall maintain and enforce policies and procedures establishing a mechanism for educating patients and caregivers about the appropriate means to dispose of expired, damaged, or unusable medications in accordance with statutes and regulations within the jurisdiction where licensed. Safety regulation #32 Safety regulation #32 Active members shall maintain and enforce policies and procedures establishing a mechanism for patients to have concerns addressed within one (1) business day (based upon the facility’s posted hours). Safety regulation #33 Safety regulation #33 Active members shall ensure that patients shall receive their medication in the time frame promised by the Active members. If a patient does not receive the order within forty-?ve (45) days, they may choose to receive a refund or the member shall have the medication re- shipped to them. At the point of sale Active members must ensure that patients are informed regarding the country where the medication is approved for sale and the country or countries from where it may be dispensed. No medication shall be stored on the ?oor. Safety regulation #50 Safety regulation #50 Active members shall ensure that all dispensing o compounding counters are neat, clean, and free of clutter.
