  2. ANEMIA HEMOLITICA • Datos de Laboratorio: • Anemia • Reticulocitosis • Hiperbilirrubinemia Indirecta • DHL aumentada • Otros: leucocitosis neutrofílica, trombocitosis • Frotis de Sangre Periférica

  3. CASO CLINICO • Paciente femenino de 22 años de edad, que acude al servicio de urgencias por presentar astenia, adinamia, fiebre no cuantificada, de instalación brusca y cefalea progresiva de predominio frontal. • A la exploración física TA en 110/70, FC 98, FR 22, temp 37 C. Palidéz de tegumentos ++, tinte ictérico leve. • Cardiopulmonar sin compromiso. Esplenomegalia leve. • Resto de exploración física sin relevancia.

  4. APP: Colecistectomía a los 18 años de edad, niega transfusionales o internamientos previos • APNP: Sin relevancia • AGO: G2 P2 AO CO niega complicaciones durante el trabajo de parto • AHF: 1 hermana con anemia hemolítica (no especifica el tipo)

  5. Reporte de Laboratorio • Gpo y Rh: O + • Hb 7.25 • Hto: 20.1% • HCM: 33.1 pg • VCM: 91.6 fl • RDW: 16.1 • Leucos: 14100 • Neutros: 80.4% • Linfos: 13.2% • Plaquetas: 301 000 • Reticulocitos: 6% AST: 69 UI/L ALT: 88 Bil Tot: 1.88 Bil Dcta: 0.31 DHL: 705 EGO Hemoglobinuria ++

  6. Se le solicita 2 unidades de paquete globular la cual da incompatible en Fase de Albúmina y Coombs ++ y +++, se le continuo cruzando unidades (10) las cuales persistían incompatibles. • Autotestigo negativo y Coombs Directo negativo

  7. Se realizó panel de anticuerpos irregulares en la que se identificó un aloanticuerpo anti-E • Fenotipo Rh de la paciente DeC/Dec
