  3. Objectives • Share My Perspective – Based on the DCU Response • What went well…. • ….and things that didn’t (Problems Encountered) • Discuss options for improvement

  4. #2 DD Incident: Sept 19, 2010 • 1.5 inch line rupture on #2 DD (Distillate Desulfurizer) • Light Vacuum Gas Oil (LVGO) release • Nature of material was lighter end, more mobile hydrocarbons • No measurable concentrations of H2S, SO2, or VOCs outside of plant • Root cause: Mechanical failure of high pressure piping

No. 2DD Response Effort • Unified Command: HOVENSA, VI DPNR Enforcement, VI DOH • UC Approved All Response Plans • Government in Oversight Role • ATSDR Provided Technical Support • EPA OSC from Puerto Rico por Un Dia • Actions Taken: • Air Monitoring During Event (HOVENSA) • Visual Assessment • All Cisterns Sampled (TPH fingerprint) • Cleaning of Property/Cars/Impacted Cisterns • Drinking Water Distribution

LP Flare Incident: Sept. 30, 2010 • Incident Summary • Slurry oil flowed into the low and high pressure flares • Black smoke and oil droplets dispersed downwind • Nature of Material: Heavier Hydrocarbon - did not wash off surfaces with rain water • Root cause: Human error; two in-line valves left partially open

  7. Profit Community Center Heaviest area found. 2mm or less LP Flare Incident - Impact Black Smoke Overhead (HP Flare) Intermittent Surge of Oil (LP Flare) Light Spotting on Surfaces – not uniform Heavier Hydrocarbon – Not Mobile No Impact to Cisterns Often difficult to find spots

  14. Delayed Coker Incident Summary: December 9, 2010 • Incident Summary • 8 minute vapor release from D-8501 ejector line that began around 0721 • Vaporized Hydrocarbon , H2S and steam (Approx. 900o F) • Deposition of Coker Residues on rooftops/cars/other structures
