RFP Section 3 – General Terms & Conditions The General Terms and Conditions contained in UTMB’s Rider 103 shall be made a part of and govern any Purchase Order, unilateral or bilateral Agreement, if any, resulting from this Request for Proposal. • Respondents may offer alternate provisions to the Terms & Conditions: • Must refer to the specific article or section concerned. • General exceptions are not acceptable. • Terms & Conditions determined to be unacceptable may result in disqualification of the proposal.   8. RFP Section 4 – Scope of Work / Service Requirements Scope of Work UTMB is looking to consolidate manufactures and/or suppliers for exam and surgical gloves. The intent of this RFP is to solicit competitive proposals for the selection of a multi award of suppliers to be used at UTMB institution. UTMB is looking for suppliers whose responsibilities include providing competitive solutions, a commitment to quality, while maintain a solid financial good standing, providing the latest manufacturing innovation in order to partner with the Institution’s forecasted growth and expansion. • Exam Gloves: Vinyl, Nitrile, Chemo tested & approve, Sterile, Therapeutic options, Latex free alternatives, Power free options, & Specialty options-multiple grip option (smooth, textured, bisque). • Surgical Gloves: Latex free alternatives, Sterile, Polyisoprene, Neoprene, Chemo tested & approved, Therapeutic options, Specialty gloves-multiple grip options (smooth, textured, bisque), Specialty options-(micro, ortho, double gloving, indicator gloves etc.) Purchasing

RFP Section 4 – Scope of Work / Minimum Requirements Service and Support • Provide standard response time to all inquiries. • Provide name & contact information for primary Contact Representatives and Account Managers. • Provide standard hours of operations for customer service. • Provide inventory levels by product line on hand. • Awarded Suppliers shall perform Quarterly Business Reviews. UTMB had consolidate distribution for medical/surgical products to include exam and surgical gloves to one primary distributor.

RFP Section 4 – Scope of Work / Minimum Requirements Minimum Requirements • Respondent’s product shall meet the following standards: FDA Regulations – Title 21 Code of Federal Regulations Part 820. FDA Regulations – AQL of 1.5 for surgical gloves. FDA Regulations – AQL of 2.5 for exam gloves. ASTM D 6978-05. ASTM F1671. ASTM Standards for Exam and Surgical gloves. ISO Certified. • Respondents shall have the technical competency required to provide assistance with all glove products. • Respondents shall provide documentation demonstrating their capability in meeting and performing all specifications of this RFP. • Respondents shall provide documentation of association or agency for which your company is listed as an approved suppliers. • Respondents shall have five (5) years of business experience with Exam and or Surgical gloves. • Respondents must have extensive experience working with academic medical center or other larger (500+bed) tertiary care hospitals. • Respondents shall three (3) past performances on similar programs with higher education and healthcare institutions of similar size and scope (academic medical centers) within the last five (5) years.

RFP Section 5 – Pricing and Delivery Schedule Pricing shall be submitted in excel format onlyAttachment A – Exam Glove Pricing Matrix Attachment B- Surgical Glove Pricing Matrix The information in both attachments shall be used as based comparison of gloves currently in use at UTMB. This is not an all-inclusive list of gloves and may be adjusted as needed.
