HISTORY PRESENTING COMPLAINTS Known case of Systemic hypertension and Diabetes Mellitus Type 2 since 1 year not on active medication c/o Right sided weakness of the upper limb and lower limb since 2days c/o fever since 2days Reddish discolouration and swelling of the left foot around shin since 2days

    3. No h/o headache No h/o dizziness No h/o vomiting No h/o Loss of consciousness No h/o any seizures No h/o fall No h/o of bowel/bladder incontinence No h/o any significant P/A ,CVS,RS symptoms

    4. Past History Systemic hypertension since 1yr not on regular medications Diabetic since 1yr not on regular medication No h/o CVA No h/o TB

    5. Personal History Sleep-normal Appetite-decreased Bowel/Bladder-normal Addiction-Smoker since 30 years Alcohol -1.5 pegs of brandy daily ,3oyears

    6. General Examination Conscious,cooperative well oriented ,thin built Vitals PR-74beats /min,RR-20/min BP-140/90mm hg Afebrile No pallor/icterus/cyanosis/clubbing/palpable lymph nodes Oedema of left foot and ankle region Skin-Reddish discolouration and increased temperature around the shin on the left leg

    7. Systemic Examination CVS- S1 S2 normal Ejection systolic murmur in pulmonary area RS Normal vesicular breath sounds,No added sounds P/A-Soft Slightly distended No tenderness No free fluid Bowel sounds heard

    8. Systemic Examination Nervous System Higher motor functions-slight slurring of speech Cranial nerves 3rd,4th ,6th-Gaze evoked nystagmus B/L.Primary gaze nystagmus Other Cranial nerves normal

    9. Continued Motor System
