TEAM DOCUMENTS • Team Documents • AZUYF 2014 Certified Roster • Waiver Status Column • OB = Out of Boundary • The OB waiver status is to be used for any player on a team who lives outside of the association's boundaries, as defined by the association zip codes, and is also an Overflow, Grandfather or Open Area player.  This player must also have an "Out Of Association Boundary Form (Non-Waiver), completed and in the team book. • EW = External Waiver • The EW wavier status is to be used for any player on a team who lives outside of the association's boundaries, as defined by the association zip codes, and IS NOT an Overflow, Grandfather or Open Area player.  This player must also have an "External Waiver" form completed and in the team book. • All other association specific codes MUST be removed from the waiver status column on the official rosters.  The only allowable codes are either OB or EW. • Certified Team Rosters MUST either be printed using LeagueWorks, or created manually using the “AZUYF 2014 Certified Roster Template” Excel spreadsheet. • Certified roster MUST be in age alphabetical order, oldest first.

  6. TEAM DOCUMENTS • Team Documents • AZUYF Attendance Form The AZUYF Attendance Form is used to record player attendance at practices during the pre-conditioning period. It must be present in the team book and complete to the date of certification.

TEAM DOCUMENTS Scholar Athlete Form • Eligible Athlete information submitted at Book Certification (The calculations of grades will take time so please try to acquire report cards (2012-2013 school year) to allow adequate time for completion of this form prior to book certification • Each business manager will evaluate athletes grades and calculate GPA or average % There will be recognition of all athletes with a GPA of 3.0 and above or an average % above 80% • First check report cards for C’s or grades below 80% at the quarter progress intervals. Athletes with these grades are not eligible for recognition • Any athletes with no C’s or grades below 80% need to have an average percentage or GPA calculated and submitted with required information on the appropriate form • See attached form document for conversion chart and specific instructions • Please make sure that all information on form is correct, especially contact information for the family.

  8. Grade Requirements/Calculations for PHX AYF 2013 Academic Extravaganza • All athletes that earn above 80% in all classes or a GPA of 3.0 will be recognized on the website. All athletes with 3.75 GPA or 87% average or above will be invited to our scholastic recognition event. This document should assist you in calculating which athletes are eligible. GPA or percentage is a converted average, but there will be no C’s accepted. You will be assessing each athlete’s 4 quarters of report cards from the academic school year 2012-2013 (last year). These should be collected for certification books. You will be responsible for calculating the average GPA or percentage, completing the form for book certification. • There are various ways to get these calculations done, so use one of the methods listed below depending on the format of each athlete’s report cards. • GPA on report cards- • 1)If your athlete has the entire year GPA on their report card-use this number. This will most likely be on the fourth quarter report card, and it will refer to total GPA. There are different ways to calculate GPA so you will still need to confirm that there are no C’s for any quarter progress grades. • 2) If your athletes have a GPA calculated on their report cards for each quarter or semester you may use that. However, you must calculate the average for the school year. This is done by adding up all GPAs and dividing by the number of time periods. For example 4 quarters- 3.5, 3.9, 4.0 & 3.6: Add them and divide by 4 = Average GPA of 3.75. Still confirm that there are no C’s for the quarter progress grades. • Grades by percentage- • Each class should score 80% average or above throughout all term periods. If there is any grade under 80% it would exclude this student as this is equivalent to a C. Add all percentages up and divide by the number of classes in order to calculate average percentage. • Calculate GPA from an A-F grading system • To calculate the athletes GPA, assign a point to each of the student’s grades. Add those points together and divide by the number of units or classes. For example grades are A+, A-, B+ & B. This equates to 4, 3.7, 3.3 & 3. Add them together and divide by 4= GPA of 3.5. Again, no C’s are eligible. • A+ = 4 • A = 4 • A- = 3.7 • B+ = 3.3 • B = 3 • For alternate grading scales or systems, please see the chart below, then convert grades to percentages and make sure all grades are above 80% for eligibility. Please be aware that there are a couple of errors are the chart. On the 9pt scale an E- = 89%. And on the 10pt scale the S=80%.

TEAM DOCUMENTS • Team Documents • Certified Team Picture Sheet • The player pictures must be in color and in certified roster order. • Each player must be clearly visible in the photo and must hold a sheet with the following information clearly visible in the picture: • Association Name • Division • Player Last Name, Player First Name • Team Name • Individual photos glued to paper will not be accepted. Each picture sheet must be a single page. (Scanned photo sheets are acceptable if they are in color)

  13. PLAYER DOCUMENTS Participant, Tracking and ID Card (Page 1) This next section is for your players, and starts with the Participant, Tracking and ID Card. All player information must be completed, use “N/A” where appropriate. Player photo (IN COLOR) MUST be in the photo section on the card. A separate photo can be attached to the form. This section is used by the association to certify that all required documents are present and valid. A league official should initial each box for approval. This section to be completed after each game with weights recorded at pre-game weigh in. This portion needs to be completed only if player does NOT make weight.

  14. PLAYER DOCUMENTS Participant, Tracking and ID Card (Page 2) You must complete all the information on the upper half of the document. Use “N/A” where appropriate.ALL parent initials and signatures must be present.

  15. PLAYER DOCUMENTS Medical Clearance Form League approved AZUYF form must be completed, signed and/or stamped by the physician. OR AIA Scholastic Physical Form will also be accepted in lieu of the Medical Clearance Form, attached to the AZUYF form. 15

  21. PLAYER DOCUMENTS Proof of Residence shall be supported by ONE OR MORE of the following categories todetermine residency of such parent(s) or guardian. Driver’s License Voter’s Registration School records Welfare/child care records Federal records State records Local (municipal) records Support payment records Homeowner or tenant records Utility bills (i.e., gas, electric, water/sewer,  Heating, waste disposal) – no phone bills Financial (loan, credit, investments) records Insurance documents Medical records Military records Internet, cable or satellite records Vehicle records Employment records

  28. Emergency Care Plan ALL COACHES/BUSINESS MANAGERS MUST HAVE COPY IN TEAM BOOK GAME DAY Game Site Street Address: Tumbleweed Recreation Center 745 E. Germann Rd. Chandler, AZ 85286 Specific directions to game site from nearest major intersection: McQueen/Germann, just south of Loop 202 PRACTICE (all teams will fill in with their information) Practice Site Street Address: Specific directions to practice sire from nearest major intersection: QUESTIONS • Where should EMS come to have quick access to the injured athlete? If emergency care is needed please have the board member on duty call 911 immediately and • direct them to the field where emergency care is needed. If at practice and no board member present the head coach and/or business manager may contact 911. • Who will give primary care to the athlete? EMS or Athletic Trainer, present • Where is the first aid kit? All teams should have first aid kit at practice field in bag. It should always include ice. On game days, the CYF first aid kit will be kept under • the Ramada by the file boxes. • Where are the emergency care cards? All business managers must have emergency medical clearance cards in team book and they must be present at ALL times, including • games and practices. • Who calls EMS? Board member on duty and/or head coach/business manager. • Who will notify the parents that the athlete is being transported to an emergency care facility? The board member on duty and/or the head coach will notify the parents. • To which emergency care facility will athletes be transported? The player will be transported to the nearest medical facility unless the parents indicate otherwise. • Who will notify the athletic administrator or athletic trainer? Once an injury occurs the Athletic trainer will be notified IMMEDIATELY and brought onto field to evaluate • situation. If no athletic trainer available, (e.g., at practice), the head coach will need to contact the league president to discuss situation. • Who will manage the rest of the team while care is given to the injured athlete? The assistant coaches will manage the rest of the team while care is being given. • Who will open any gates or doors for EMS? The board member on duty and/or head coach will watch and direct the EMS to the proper fields. • Who will follow-up with the parents? The league president will follow up with all involved parties, including parents, EMS, athletic trainer and head coach. • Who will document the injury? All teams must fill out proper forms to document injury. If a medical clearance form is needed the team business manager will provide that • document and keep it in the team book. • Who will speak to parent in the instance of catastrophic injury? The league president along with 2 board members will speak to the families in the event of a catastrophic • injury.
