  2. 75 year old male with recent history of subdural hematoma presents with increasing left arm weakness • Noncontrast CT done for evaluation

  3. Differential • Air introduced iatrogenically • Gas producing organism within an abscess

  4. Findings • Evidence of craniotomy (to evacuate hematoma) • Subdural Hematoma • Subdural and subcutaneous air • Effaced right fronto-temporal sulci • Slight right to left midline shift

  5. Discussion • Intracranial air (pneumocephalus) only seen on CT or X-ray, not MRI • Clostridium perfringens is the most common gas-producing bacteria found in brain abscesses
