  5. 攜帶證件（全班申請） • 班代至系辦領取通過名單→膳打全班之基本資料(含姓名、學號、補助金額…等) →待回班上請各位同學簽章→繳回系辦。 • 連同前頁敘述之專業證照申請表一併繳交，即完成全部作業。

  6. 攜帶證件（個人申請） • 證照正本或影本 • 身分證字號 • 繳費單正本 • 專業證照申請表 • 印章 • 預轉入申請經費之帳戶號碼：銀行名稱及分行名稱
