  1. CDP Portal Forms Ordering

  2. You will continue to order all WIC forms from the Bridge/GUI screens. The WIC forms will NOT be available on Portal at this time.

  3. To Order forms, on the CDP Portal page, click on Forms Order Search

  4. Forms Order Search Page

  5. First select your District or Clinic from the available drop downs

  6. Form Number and/or Description can be filled in to narrow your search. Next hit the Search button.

  7. Search results will be returned. Find the form you want to order and click the green arrow next to the Form Number

  8. The Forms Order page will comes up. It will have the email address of the person ordering and the name and address of the clinic that is ordering the forms pre-filled.

  9. The person requesting the order will need to fill in the quantity and a phone number that they can be reached.

  10. Order restrictions will be shown on the right hand side of the page.

  11. For this form, 5 is the minimum order, 50 is the maximum. Each package contains 5 forms.

  12. When you are placing an order, remember you are ordering by the number of FORMS not PACKAGES.

  13. For this form, you can order in increments of 5. If you enter a quantity that is not an increment of 5, the page will round the quantity ordeded up to the next increment.

  14. Once you have quantity and phone # completed, click SUBMIT. This forms order is ordering 5 forms.

  15. After submitting, a message will tell you if the order was accepted.

  16. An email will be sent out letting you know the order was placed.

  17. If the form does not have any order restrictions, any amount can be ordered.

  18. Don’t make a mistake when ordering.Because for now, there is no way to delete the order in the system.

  19. You will continue to order all WIC forms from the Bridge/GUI screens. The WIC forms will NOT be available on Portal at this time.
