Springs Community Night SchoolStudent Enrollment ProcessDecision flowchart for School Counselors YES NO Direct the student to the SCNS Web site or Office (328-2160) for Enrollment Information. • If you recommend that the student supplement their daytime education by taking courses at SCNS, complete the Dual Enrollment form. • Both sidesof the Dual Enrollment form need to be completed and signed for processing. • NOTE: The Day Counselor determines class(es) to be taken by completing the “Classes Needed” section of the Dual Enrollment Form . • (Please note if the student attends an out of District school or a charter school they must also complete the D11 New Student enrollment forms.) Will the student be dis-enrolling from your program to attend SCNS? Student and Parent/Guardian will work with the SCNS staff to complete the Enrollment Process Please provide to students a copy of their unofficial transcript and immunization records if you are recommending or they are seeking full time enrollment in our program NOTE: The Night Counselor at SCNS determines the classes to be taken. 4/4/12
