  1. On a Seamless Transition from a Running EPR System to a New One M.Shifrin N.N.Burdenko Neurosurgical Institute Reykjavik, 2,June, 2010 EFMI STC 2010

  2. N.N.Burdenko Neurosurgical Institute (R&D)of the Russian Academy of Medical Sciences • Established in 1932 • Now: new surgical building (1999) • 300 neurosurgery beds • 40 beds in ICU • 14 operating rooms • Radio surgery department • Neurorehabilitation department • Full set of diagnostic and laboratory equipment • ~5500 admissions/year • Out-patient department

  3. Electronic Patient Record System e-Med

  4. Evolutionary development 2000 – 2010 “spatial” “cognitive” “functional”

  5. Evolutionary development (cnt.) • “Spatial” dimension: • 1 working place  ~ 400 • 4 users  ~ 600 • “Functional” dimension: • 1 user function (admission/discharge)  ~ 150 • “Cognitive” dimension: • free text  adequate formalization

  6. After 10 years of "smooth" evolution…

  7. Following Hamburg score…

  8. "Inborn defects" Transition to industrial system

  9. Objective difficulties Integration

  10. Transition Difficulties "Transitional" evolution

  11. Principal Requirements to the Transition Process Transition platform

  12. Industrial system+ Integration+ Transition platform Integration platform

  13. EPR Old Other systems Integration platform

  14. EPR Old New Other systems Integration platform

  15. EPR Old New Other systems Integration platform

  16. EPR Old New Other systems Integration platform

  17. Advantages • Seamless transition without halting business processes • "Best of Breed" • Possibility for permanent development • Creating of integrated information environment • Openness to SOA and SaaS ideas

  18. Last but not least • A hope not to meet transition problem in 10 years • – transition will be permanent

  19. Difficulties • Need for a new view at the existing system, adequate to the transition process • Transition gives a chance for reengineering of business process – not to miss it • Transition in complex with implementation…

  20. Pilot project: e-Med – TrakCare Lab EPR LIS e-med Orders Orders Results Results Ensemble Buffer DB (MS SQL) Laboratory devices TrakCare Lab

  21. Results of the Pilot • It works! • Transparent integration was realised • The integration required minor changes in the programme code of the e-Med system • A prototype of "transition protocol"

  22. Next steps • To select a new system • To elaborate complex implementation-transition plan. • To realize it!

  23. Thank you for your attention! shifrin@nsi.ru Reykjavik, after the STC opening.
