Appreciative Moment • Tell us one thing that you learned from your work on sepsis that you would want to spread to rest of HEN hospitals. • St. Francis Hospital is going to share their story of a “work in progress” toward Sepsis mortality reduction • Please welcome the St. Francis Team: • Julia, Angie, Paige, Susan, and Tammy • St. Francis Hospital Columbus, GA

  3. Making Time to Meet • St. Francis utilizes a Pneumonia/Sepsis/COPD team that already meets frequently • Time for meeting is split between diagnoses and sub-committees are utilized when immediate attention to one areas is required

Our Bundle – Handout provided • Paper versus Electronic Process • Updating orders to assure antibiotic compliance occurs • Working on early goal directed therapy – recognition/screening of Sepsis, treatment and appropriate/correct documentation of Sepsis, ID as Sepsis with Pneumonia Secondary • Identifying opportunities for Education for Nursing Home physicians re: Palliative care • Developing Sepsis education plans for nurses, physicians and community

The Spread • CMS has asked us to spread beginning in June • Introduction of spread challenge at Regional Meetings • Presentation to CMS CMO and other CMS leaders May 30 • Spread begins at Nurse Leadership Institute Jun 4 – 6 • All LEAPT hospitals need to gear up now to address all topics • What questions/concerns are there about Spread?

  14. GAPP Updates • Mandatory Meeting Attendance • Coaching Call and Sepsis Discussion Second Thursday of month. Must complete an evaluation • If you miss a call you can listen to the recording within 1 week. Complete eval if applicable, and notify topic lead that you listened to get credit. • Data Submission: • Due 3rd or each month – send to Lynne Hall (lhall@gha.org) • TOC (1 for sepsis and 1 for additional topic area) • Checklist (1 per hospital) • Worker Safety Data (if in WS group) Send to Jean Allred (jallred@gha.org) • due 15th of month (about 45 days after end of reporting month). • Next Coaching Call and Procedural Harm/Failure to Rescue Working Session July 17, 1 -2 pm
