  4. Alle patiënten van 1 huisarts werden toegewezen aan zelfde diabeteseducator 3 diabeteseducatoren Diabeteseducator contacteerde patiënt voor 1ste huisbezoek Overdracht patiënt

  5. Contact met huisarts Toelichting van het project Afspraken ivm informatie-overdrachtsformulier Doelstelling van de educatie Feedback tijdens educatiesessies

  6. 1ste huisbezoek Persoonlijke kennismaking met patiënt Bespreking verloop project Opmaak diabetesdossier adhv informatie-overdrachtsformulier Afname kennistoets Opmaak kalender: planning van de educatiesessies
