Legend • Green = Collected 80%+ of required screens. • Yellow = 50-79% of required screens. • Red= <50% of required screens. • Mechanical screens are required at baseline and every 6 months until discharge. • Lab screens are required every 12 months starting at baseline or 6 month reassessment.
