  4. Medication Incidents • The numerator is the number of medication related incidents that have caused serious harm to a patient. • Serious harm = MERP categories F-I • Incidents reported thru online system and mortality review • The denominator is 1,000 patient days.

Serious Harm • The numerator for the rate is the number of incidents that have caused serious harm to a patient. • MERP categories F-I and NDNQI categories for harm from Falls, Major and Death • Incidents reported thru online system and mortality review • The denominator is 1,000 patient days.

  6. Reportable Adverse Health Care Events • The numerator for the rate is the number of reportable adverse events. • Types of events reportable under MN statute • Incidents reported thru online system and mortality review • The denominator is 1,000 patient days.

Preventable Harm • Failure to provide care to the existing standard or where a reasonable adaptation to the existing standard will likely decrease the risk of future injury by the same mechanism. • Falls, pressure ulcers, medication related, failure to rescue the deteriorating patient, transitions of care, central line associated bloodstream and surgical site infections, surgery or procedure related. • Number of days between preventable seriously harmful events
