Health Impact Assessments Definition: …a combination of procedures or methods by which a policy, program, or project may be judged as to the effects it may have on the health of a population (WHO Regional Office for Europe, 1999) Key Question: how do decisions affect our health? Traditional public health science (epi) searches for disease causes HIA aims to evaluate how social decisions can impact health Value Focuses attention of decision-makers (usually with no health background) on the health consequences of policies Contributes demand for info on health consequences in policy/planning Strengthens actions to prevent health harms (not just responding)

Common Health Impacts of Ports Air Pollution in the form of ozone, small particulates, and hazardous pollutants such as from diesel engines at ports can cause asthma and other respiratory diseases as well as cardiovascular disease, lung cancer, immune system disfunction, and premature death. Diesel exhaust has 40 different HAPs that are toxic, mutagenic, or teratogenic. Noise Pollution. Traffic and construction activity cause environmental noise: Container loading is 65 to 79 decibels, Rail crossing guards are 95 decibels. Noise can increase hypertension, blood pressure, and heart disease. Exposure to environmental noise is associated with delays in learning like impaired cognitive development in children, such as reading comprehension, speech intelligibility, and memory. Long term environmental noise exposure can cause sleep disturbances and hearing impairment. Environmental noise can also impact concentration, increase aggressive behavior, and decrease helping behaviors.

  7. Light Pollution. Night time operations at ports and other nearby facilities emit light pollution from large banks of lights (can be 12 stories high or more), which shine on nearby residential properties and pass through curtains, and reflect off water. This can cause chronic sleep disturbance, which can reduce immune system functioning, reduce concentration, increase aggressive behavior and decrease helping behaviors. Water Pollution. Water pollution from port activities can impact the health of marine wildlife and humans. TBT, used to prevent barnacles and other marine organisms, is linked to skin irritation, stomach aches, colds, flu, and neurological symptoms. Oil from bilge and other ship-related secretions harms ecosystems and human health, while dredging ports stirs up toxic substances which affect neurological systems, cognitive development, and chronic disease.

Traffic in and near ports increases vehicle collisions involving pedestrians and reduces walking and cycling on residential streets, thus reducing physical activity and increasing obesity, heart disease, cancer, and early death. Involuntary displacement due to increased port capacity or activity can result in loss of jobs and protective social networks, depression and lowered mental health, stress, child abuse, homelessness, and social cohesion. Employment conditions. Positive health effects due to economic prosperity of local residents only occur if well-paying jobs are held by residents. Construction jobs are short-term and have little lasting community impact. Working conditions, too, must be safe and monitored in order to preserve the health or workers. These affect acute and chronic physical health conditions, as well as mental health. Neighborhood Livability Effects. To the extent that ports are perceived to diminish quality of life, they will lower a city’s potential to develop livable neighborhoods, increase social cohesion, and stimulate the local economy, which can lead to increased injury and morbidity from crimes, stress related illness, and increased risk or injury or death from reduced city services. Some ports operate 24 hours a day, 7 days per week

The experience of other communities could be instructional People who live in 327 homes located in the Bayport noise-mitigation zone are in the throes of trying to decide whether to accept the Port of Houston's offer of $40,000 per home owner, in exchange for granting the port an easement on their property. Key Questions to Decisionmakers 1. Can the developers provide an example of a port they have developed that is situated near residential and tourist areas that would serve as a model? 2. How much of the port revenues would benefit local government? 3. How much funding would be allocated by the Port for mitigation and community benefits, and who would have control of those funds? 4. Will special use permits be used to limit the health impact of port expansion or development? 5. What are the projections for jobs benefits within the local economy (not including short-lived construction jobs)? 6. What recourse measures will the community have to ensure a positive impact on the community?

  12. Ways to measure and monitor health impacts within port communities continue to develop, creating an opportunity for establishing widely shared standards not only for measuring and monitoring, but also for developing acceptable standards and practices by ports.
