  3. WHAT IS A CONCEPT MAP? • “Schematic devices for representing a set of concept meanings embedded in a framework of propositions” (Schuster, 2002, pg. 2) • An approach to planning and organizing nursing care

  4. Step One • Develop a basic skeleton diagram Key problem Reason for Health care

  5. STEP TWO • Analyze and categorize data • Add clinical data to the appropriate problem boxes • Identify key assessments related to the patient’s reason for seeking health care

  6. STEP THREE • Analyze nursing diagnoses relationships • Draw lines between related problems • Number the problems in order of priority

  7. STEP FOUR • Identify goals, outcomes, and interventions

  8. STEP FIVE • During post-clinic session, evaluate your patient’s responses to the identified nursing interventions

  9. GATHERING CLINICAL DATA • Chapter 2 in textbook • Standards of Care • Important data • Patient care plan or data base
