INCAPICATED CHILDREN • Although considered an adult at age 18 military family members remain eligible till age 21. • Children over the age of 21/23 may be eligible to continue benefits under certain conditions. • Program is managed by the Sponsor’s Personnel Department. • Guidance for completing application can be found in AFI36-3026(I) Section 4-6 BR400801BEN0504C

  3. ELIGIBILITY CRITERIA • Unmarried • Incapable of self-support because of mental or physical incapacity that existed before the child’s 21st birthday or 23rd if enrolled as full-time student. • Child must be dependent on the sponsor for over one-half of his or her support or have been at time of sponsor’s death.   4. DOCUMENTATION • Current physician’s statement (dated within 90 days of application) • If over 21 a Physician’s statement indicating the incapacitation occurred after the 21st birthday but before the 23rd. • Letter from school registrar reflecting the dependent was enrolled as a full-time student in an accredited institution of higher learning when the incapacitation occurred.   5. HOW TO APPLY • Complete Application DD Form 137-5 • Navy Personnel send application and all related documents to: PERS 312D Navy Personnel Command 5720 Integrity Dr Millington TN 38055-3120 (901) 874-3360 DSN-892-3360

  6. HOW TO APPLY • All other Service members contact your Personnel Office for information on where to mail you completed paperwork.

  7. APPROVAL COMES FROM • Army- DFAS-Indianapolis • Navy-Bureau of Naval Personnel • Air Force- DFAS-PMJPD/DE • Marine Corps- Letter from BUMED • Coast Guard- Letter from Human Resources Service and Information Center • NOAA/USPHS- Letter from Uniformed Service Personnel Project Office BR400801BEN0504C

  8. HOW TO GET THE ID CARD • Once approval letter is received • Complete DD Form 1171 and go the to nearest ID Card Issuing facility with the completed DD1171 and your approval letter.
