  6. If the answer is YES... Please let a Receptionist know if you would like to attend. Monday 22nd July at 10:30am

  7. Would you like to play a part in shaping the services here at Omnia? ATTENTION ALL PATIENTS!

  8. Then your seat awaits you at our next PATIENT GROUP MEETING Thursday 15th August at 6:30pm Let A Receptionist Know If You Would Like To Attend.

  9. Would You Like To Join Our Patient Group but have no time to attend meetings?

  10. Then join our VIRTUAL GROUP Just leave your email address with a Receptionist and we will update you by email.

  12. Take a LOOK at our noticeboards. There's lots of useful information there.

  13. Are you on specialist medication that has been supplied by another organisation? This could be a hospital or another health organisaton.

  14. If you are, please check that our Doctors or Nurses know so that it can be recorded on your notes. This is to reduce the risk of interactions. We call this Medication Reconciliation.

  17. There has been a change to the Health Visitor drop in clinic. The ‘drop in’ clinic will now only be open Thursday 1:00pm -3:00pm. CHILD IMMUNISATION IS YOUR CHILD UP TO DATE?

  18. IMMUNISATION IS VITAL Please check your ‘Red Book’ to ensure your child’s immunisations are up-to-date.

  19. CONFIDENTIALITY We take this very seriously.

  20. We cannot give out any information about our patients to anyone.

  22. Please check with Reception or complete a form. Sometimes we need to call you to give you important information.

  27. OPENING HOURS Monday, Wednesday & Thursday 8:30am to 8:00pm Tuesday & Friday* 8:30am to 6:30pm We are closed between 1pm and 2pm on Fridays for Staff Training
