1. What is prilocaine?
Prilocaine is a local anesthetic(prilocaine local anesthetia)of the amino amide type
first prepared by Claes Tegner and Nils Löfgren. In its injectable form , it is often
used in dentistry. It is also often combined with lidocaine as a topical preparation for
dermal anesthesia , for treatment of conditions like paresthesia. As it has low cardiac
toxicity, it is commonly used for intravenous regional anaesthesia (IVRA).
2. Raw material of prilocaine
Raw material of prilocaine is the main ingredient for making prilocaine injectable
form or prilocaine cream form. Prilocaine powder is white crystalline
powder, Prilocaine powder/prilocaine local anethetia is chemically designated as
propanamide, N-(2-methylphenyl)-2-(propylamino), has an octanol: water partition
ratio of 25 at pH 7.4, and has the following characteristic:

3. Prilocaine injection form and Prilocaine cream form?
Prilocaine injection form
Prilocaine Injection form,is a sterile, non pyrogenic isotonic solution that contains a
local anesthetic agent and is administered parenterally by injection. Prilocaine
Injection, contains prilocaine raw powder, which is chemically designated as
propanamide, N-(2-methyl-phenyl) -2- (propylamino)-, monohydrochloride.
Prilocaine stabilizes the neuronal membrane by inhibiting the ionic fluxes required for
the initiation and conduction of impulses, thereby effecting local anesthetic action.
Maxillary infiltration: initially 1–2mL; max recommended dose within a 2-hour
period: <70kg: 8mg/kg; ≥70kg: 600mg. Elderly, debilitated, acutely ill: reduce dose.
<10yrs: use Clarks rule to calculate dose.
Congenital or idiopathic methemoglobinemia.
Interactions: Risk of methemoglobinemia with sulfonamides, acetaminophen,
acetanilid, aniline dyes, benzocaine, chloroquine, dapsone, naphthalene, nitrates and
nitrites, nitrofurantoin, nitroglycerin, nitroprusside, pamaquine, para-aminosalicylic
acid, phenacetin, phenobarbital, phenytoin, primaquine, quinine.
Prilocaine cream form
Prilocaine cream form is an emulsion in which the oil phase is a eutectic mixture of
lidocaine powder and prilocaine powder in a ratio of 1:1 by weight. This eutectic
mixture has a melting point below room temperature and therefore both local
anesthetics exist as a liquid oil rather than as crystals. It is packaged in 5 gram and 30
Lidocaine-Prilocaine cream contains lidocaine powder 25 mg, prilocaine raw powder
25 mg, polyoxyethylene fatty acid esters (as emulsifiers), carboxypolymethylene (as a
thickening agent), sodium hydroxide to adjust to a pH approximating 9, and purified
water to 1 gram. Llidocaine-prilocaine Cream contains no preservative, however it
passes the USP antimicrobial effectiveness test due to the pH. The specific gravity of
EMLA (lidocaine and prilocaine) Cream is 1.00.
4. How long does prilocaine last?
Excellent skin numbing results are obtained 30 minutes to 1 hour post application.
Don't be impatient: give the product time to work! That way, you obtain the most
benefit out of it.
Please don't fall for other products that promise "instant numbing". Unless the skin is
broken, is not going to happen.
That said, some areas of the body numb faster than others. For example, if you put
some of the gel on your lips (the red part), you will feel numb - really numb - in less
than ten minutes. That is because the skin there is very permeable. Try it yourself,
once you get the product, and you will believe it!
However, most other areas of the body take more time, because the skin elsewhere is
thicker, with many keratinized layers that act as a barrier.
TOPICAINE® is formulated to overcome this barrier, because it contains skin
permeation enhancers. These are substances that temporarily and reversibly increase
the permeability of the stratum corneum - the skin barrier.
In addition, the product needs some time to penetrate to where the pain receptors are
located, deep into the dermis. In some areas, like the lips, the numbing action can be
felt much faster because the nerve receptors are located very superficially. Most other
locations of the body the receptors are located deeper, and it takes longer to reach
If the application time is 30 minutes, the skin numbing effect will last approximately
1 hour. If you apply the product for 1 hour, your skin will remain numb for about 2
hours, even after you remove it!
As a rule of thumb, remember this:
the duration of the numbing is about twice as long as the application time, and
the longer the application time, the longer and deeper the numbing effect
