Identify and Invest in Promising Approaches • Economic Model • In-person caregiver support • Disseminated summer-fall 2013 via website, partners • Influence payment policy to ensure optimal care and supports are provided in a way that reduce the State’s and other payer burden ** Funders: BCBS; Care Providers of MN Foundation; UCare

Increase Detection and Improve Care • Disseminate provider tools • Smart Set for inclusion in Electronic Medical Records • Dementia Curriculum Adoption and Next Generation • Embed tools and Smart Set so optimal care is provided • Embed curriculum to ensure professionals are prepared ** U of MN Center on Aging (HRSA funds)

Sustain Caregivers • MinnesotaHelp.info search feature enhanced to find housing and services that indicate capability in dementia or memory care • Dementia capable Senior LinkAge Line staff and Caregiver Coaches • State caregiver support is dementia specific **Leveraging DHS Systems Integration Grant Funds

  6. Equip Communities and Raise Awareness • Refining Dementia Capable Communities Toolkit • Supporting five pilot communities • Developing community engagement technical assistance plan *Toolkit development funded by GreaterTwin Cities United Way

ACT on Alzheimer’s: Second Half and Next Generation • What can we do to assure continued engagement and investment in ACT on Alzheimer’s through 2015 and how do we distinguish natural movement from waning engagement? • While we anticipate that the ACT on Alzheimer’s architecture will transition at the end of 2015, what should we be planning for in terms of sustaining the efforts of the initiative long term?
