Report Requirements: Content • See cross walk document • All CHA-CHIP requirements must be documented in report, e.g.: • Model and process adopted • Partnerships • Data sources • Health measures + social determinants • Disparities • Priority setting criteria • Implementation and marketing strategies • Measurable health outcomes in CHIP

  3. One Report or Two? • No clear guidance • Pros and cons

  4. Recommended Sectionsof a CHA-CHIP Report • Background • Describe community • Methods • Results (“profile”) • Strengths, challenges, P.H. issue statements • Planned actions (CHIP) • Planned follow-up and implementation monitoring

  5. Public Health Issue Statements • Useful to explain in simple, neutral terms P.H. issues • Contains: • Statement of main issue • Statement of contributing factors (“due to…”)

  12. Next Steps • Was the format of one centralized learning collaborative program effective? Should we use it again? • Was the format of involving regional teams effective? Should we use it again? • Was the duration of the LC (3 sessions) and of each session adequate? • What topics for face to face peer to peer sessions would you like to address? • What other forms of TA would you like the project to provide? • Are you willing to provide TA to peers in other LHDs? If so, would you requires reimbursement for that?
