Sussex Partnership NHS Foundation Trust is committed to improving the experience of Care Home residents with dementia, through delivering a highly specialised, quality, Care Home In-reach service in conjunction with service users and their carers, GPs, PCT and LA colleagues and representatives from acute hospital Trusts and Care Homes Introduction

  3. To reduce the amount of unplanned admissions to acute care and any moves to more intensive care settings e.g. from RH to NH. To improve the quality of care for the person with dementia by providing training to care staff in person centred/psycho social/ occupational /environmental/diversional approaches. To avoid/reduce the use of anti-psychotic medication for those people with dementia who have “challenging” behaviour. Purpose

  4. To ensure that prescribing and administration of such medication is appropriately reviewed. To develop positive working relationships with care homes. To effect long term improvement through support and training. Purpose

  5. Each person will be: Given respect for individual qualities and diversities. Entitled to best quality care in the best possible environment. Acknowledge that older people and those with cognitive difficulties respond well to stability, familiarity, structure and continuity. Provided with safe, supportive intervention through collaborative relationships. Actively engaged, where possible, in care planning, delivery and evaluation of care. Care staff will be supported to provide best possible care through training. Philosophy

  7. For people with suspected cognitive impairment. For people living with a formal diagnosis of dementia. For people who are resident in a residential or nursing home provided by the independent sector. Eligibility

  8. One to one meeting with the Manager of the home. Self Assessment form/Demographic form completed. Subsequent meeting with staff team to discuss SA form and determine Action Plan. Provide input tailored to requirements of each individual care home. The Process

  9. Work with care homes to provide support in developing the skills of care staff. Identify individual residents who would benefit from specialist input relating to the use of anti-psychotic medication by working closely with the Psychiatrist, Pharmacist and GP. Support care staff to access training already available and provide information workshops on topics identified at the SA/Action Planning stage Support staff to translate theory into everyday practice by use of workshops, role modelling, care planning, networking and facilitation of reflective practice discussions.
