  2. 兩件骨髓移植輸血病安事件 案1：病房退回A病人(病歷血型A型)血小板與冷凍沉澱品各12U，退血原因為該病人是骨髓移植後之病人，需改輸AB型血小板。但兩次病房開立之輸血單均未在輸血單之注意事項欄位或空白處註明需AB型血品。 案2：病歷血型O+之骨髓移植病人需輸血減白血小板 1袋，輸血單特別註記「O+ to A+需照射處理」，導致血庫發血錯誤，而血品也因出庫過時而以報廢處理。醫師表示，病人要輸AB型血小板，但因註記欄位只能寫20個字，故僅填「O+ to A+需照射處理」，認為血庫應該理解即為AB型血小板。

  13. 血型檢驗困難實例 1 • 背景：接到某病人輸血申請，依血庫作業進行試管法 ABO 血型檢驗。 • 問題：在試管中，anti-A與血球反應為mixed field(混合試驗)，而有cell typing和serum typing結果不符現象(ABO discrepancy)。 • 依常見的實驗室作業解決ABO 血型不合問題 ，但無法獲得確定結果。 • 經詢問病史得知病人常接受BMT

When to swith the Blood type ? • Major ABO incompatible HSCT: • Forward typing: donor type • No more detctable recipient derived agglutinins directed against donor RBCs • Negative direct antiglobin test (DAT) at least on two separate occasions. • Minor ABO incompatble HSCT: • Forward typing: donor type • Reward typing: no correlating with forward typing (ABO discrepancy) • Detection and management of relapse
