Förbättringsområden- VPL Förberedd och delaktig Esther Närvaro - Innehåll Avskild, bokningsbar plats • Ett samordnat möte(VPL) som inför Esthers utskrivning från slutenvården ska skapa förutsättningar för att Esther ska känna trygghet. Mötet ska klargöra Esthers behov av vidare insatser och utgöra en planering för vem som ska stå för vilka insatser.
