  2. Bree Collaborative Topics • Readmissions • Spine/low back pain • Cardiology • Obstetrics Workgroup & subgroup membership posted on the Bree Collaborative website: http://www.hta.hca.wa.gov/bree_members.html

Potentially Avoidable Readmissions (PAR) • PAR workgroup meeting #2 held 9/5/12 • Reviewed and adopted work plan • Heard presentations on WA readmission initiatives • Meeting #3 - Mid October • Start prioritizing care transition strategies for individual health systems and the community • Start reviewing report outline

Readmissions Accountable Payment Model Subgroup • Potential nominees solicited from Bree members, WSHA, WSMA, and other stakeholders • Nominees for sub group adopted by Bree steering committee in late September • In the process of asking nominees to serve/finalizing group • Payment Reform/Bundled Payment Webinar by national experts Harold Miller and Francois de Brantes on October 12th

Spine/Low Back Pain • Bree Collaborative at its August 2nd meeting approved a motion to form a workgroup to develop evidence-basedrecommendations on appropriate management of acute low back pain (under 3 months) • Nominees for workgroup adopted by Bree steering committee in late September • In the process of asking nominees to serve/finalizing group • First workgroup meeting – to be scheduled for end of October/beginning of November, followed by monthly meetings

Cardiology RECAP • Wide variation of use of percutaneous coronary interventions (PCI); use of PCI for acute MI is appropriate; not appropriate for non-acute or elective procedures • “Appropriateness” hard to measure because insufficient/missing data collected by hospitals; COAP’s goal: help hospitals reduce inappropriate procedures by helping them reduce insufficient data • Bree Collaborative in July asked COAP to publically report appropriate use of PCI data on a quarterly basis, by hospital • COAP approved BreeCollaborative’s request and 4-step process & timeline to fill missing PCI data and publish

Cardiology, Con’t Status: On schedule Step #1: Completed, August 1, 2012; Appropriate use insufficient information report (based on Q2 2011 through Q1 2012), by hospital, was posted on COAP members-only section of COAP website Step #2: In process; COAP staff working with hospitals to provide feedback and tools to reduce the amount of insufficient information in their data, by end of December 2012 • COAP staff met with one hospital that requested assistance • Goal of COAP staff: reach out and meet with every hospital before the end of December 2012

Cardiology, Con’t To be completed in 2013 Step #3: An updated appropriate use/insufficient information report (based on 4th Quarter 2012 data only), by hospital, given to the Bree Collaborative and hospitals to review; hospitals will have the option to not be identified; Target date: April 15, 2013 Step #4: An updated report (based on 4th Quarter 2012 data only) will be posted on the public section of the COAP website and potentially the Puget Sound Health Alliance ‘s Community Checkup website, on a quarterly basis; hospitals will have the option to not be identified; Target date: May 1, 2013

Obstetrics Recap: Bree adopted OB report & OB Implementation Plan on August 2nd • Report sent to HCA administrator on August 21st • Per Bree legislation HCA administrator reviews and decides to apply recommendations to state purchased programs (Medicaid, PEBB, Dept of Corrections, L&I) • HCA administrator reviewing report now; outcome of review most likely announced in a few weeks • “Implementing” Implementation Plan • Started disseminating report to Stakeholders • Hospital CEOs (via Dept of Health) • WA Perinatal Collaborative (via Dept of Health) • National experts • Highlighting report in organizations’ newsletters/blogs • Puget Sound Health Alliance newsletter • State of Health reform blog

  10. Obstetrics, Con’t Implementation Tasks for October • Continue disseminating report to stakeholders • Continue to promote report (e.g., Seattle Times, blogs, newsletters, etc) • Organize media event with Governor
