NAME and DOB (or Medical Record Numberfor Members with identification band) e.g. (a) Administering medication, blood or blood components (b)Collectingblood samples and other specimen (c) *Labeling containers for blood and other specimen in the presence of patient (d)Providing any treatments or procedures Comply with current Centers for Disease Control and Prevention (CDC) hand hygiene guidelines. WASH YOUR HANDS - 15 SECONDS Use Proven guidelines to prevent infection after surgery 3. Use Medicines Safely Prevent Mistakes in Surgery LABEL all medications, medication containers (e.g. syringes, Medication cups, basins), or other solutions on and off the sterile field Reduce harm from ANTICOAGULATION therapy RECORD and PASS ALONG correct information about a patient’s medicines: e.g. (a) What medicines the patient is currently taking. (b) Compare those to new medicines given to the patient. (c) Patient knows which medicine to take when at home (d) Patient aware of the importance of bringing up-to-date list of medicines for every office visit with a doctor Make sure that the correct surgery is done on the correct patient and at the correct place on the patient’s body, e.g. Conduct preoperative verification process with the patient, e.g. MARK the OPERATIVE SITE & conduct “TIME OUT” immediately before starting the procedure. Remember to Use: “SAFETY CHECK” to Ensure Safe Care
