Sustainability and Transformation Plan • “Delivering the Forward View” signalled the requirement for a five year, place-based ‘Sustainability and Transformation Plan’ (STP) to be co-designed and co-produced by commissioners and providers • The STP is an umbrella plan covering three Local health economy plans on different geographic footprints. • The plan is about delivering the Five Year Forward View and closing the three gaps, namely: • the health and wellbeing; • the care and quality; • the finance and efficiency gaps.

  3. Northumberland, Tyne and Wear Sustainability and Transformation Plan The Northumberland Tyne and Wear STP footprint is a new collaboration covering a total population of 1.5 million residents across three Local Health Economies (LHEs): • Newcastle Gateshead • Northumberland and North Tyneside • South Tyneside and Sunderland Organisations delivering Health and Social Care within the STP footprint are detailed on the map; including North Durham due to patient flows and geography.

  4. The Vison: A place-based system ensuring that Northumberland and Tyne and Wear is the best place for Health and Social care

  5. Common areas of work • Prevention • Out of Hospital working • Primary care and community transformation • Advanced practices • Reducing variation in outcomes and referrals • Workforce, Capacity, Estates, Technology

  6. Progress to date • Submission(s) of the plan to NHS England with increasing detail required • Governance structure developed • Identification of transformation areas for the STP that will support reducing ill health and demand and how we will do so in a safe and sustainable way • Prevention, Health and wellbeing • Out of hospital collaboration • Optimal use of the acute sector

  7. Next Steps • Discussions with NHS England about the plan • Local Health Economy plan, South Tyneside and Sunderland, being developed along side the NTW plan • Detailed South Tyneside level plans in the process of being developed
