5. Illinois 4-H Award Application • For National 4-H Congress & National 4-H Dairy Conference • Covers ALL years of 4-H participation • Submitted in specific program area • Complete ONE Summary of Accomplishments • Submit a different 4-H Story for each area

11. Size and Growth • Size is anything that can be expressed in quantity: • Took 360 photos in 2011 • Earned $300 babysitting in 2010 • Growth shows a progression from the first year to the present • - Took 100 pictures in 2010; 250 in 2011 • Progression from simple to more complex (made after school snacks and now make entire meal for family) • Advancement through different project levels (Sewing I to Sewing III)

12. Knowledge & Skills • Show a progression of skills from simple to complex • Learned to use a hand saw and hammer; select the correct type of nails; use a power saw; use a miter box to cut angles; use a router; design a chest and figure the lumber needed • Don’t need to list everything, but need to show progression of skills

13. Levels of Participation • (L) Local – club level • (C) County – county held events/workshops • (M) Multi-County – 4-H Camp • (R) Regional – Space Camp • (S) State – Illini Academy • (N) National – 4-H Congress • (I) International – foreign exchanges

14. Demonstrating Leadership • Leadership means taking the lead by: • Giving direction • Planning • Organizing • Mentoring younger 4-H member • Use active verbs & include numbers • Include how many you taught, led, scheduled, etc. • Trained 4 members in tool safety • Taught 16 mini campers to make leaf prints • Organized club foods workshop for 12 members

15. Leadership “words” Be sure to use a variety of words to show your leadership: • Organized (club food drive) • Taught (nature campers to make leaf prints) • Led (club recreation activity) • Supervised (Cloverbud campers) • Arranged (supplies for workshop) • Planned (community service activity for club) • Judged (beginning 4-H foods projects)

Community Service • Acting with concern for others as an individual or part of a group • Helping and assisting counts here • Community service projects • Do not duplicate items listed under Leadership • ONLY 4-H/Extension-related items should be under this section; School & Community projects should be under their specific section

17. 4-H Story – 25 pts • Be Creative, but Factual • Look at criteria& scoring for the story and use that information to form your outline • Include something in story for each item being scored • Include the impact 4-H has had: • - Successes and Failures – What knowledge did you gain & how did you use it? • - On you and your family • - On your career choice & future plans

18. READ & FOLLOWthe Directions! • Paper, Font Size (12 pt.), Margins (one inch minimum) • ONE Summary of Accomplishments • Maximum of 4 pages for Summary • Maximum of 2 pages for each 4-H Story (use two pages . . . Double space between paragraphs, make heading larger so it IS two pages in length)
