  1. Corporate Card Application Guide • Complete the ‘AMEX Corporate Card Application Form’ via Australia URL Link on the HP Travel website (http://corp.americanexpress.com/gcs/clients/hp/region/docs/japa/AustraliaCardApp.pdf ).Please note that you can complete the form on screen and then print it for signing and sending. • Guideline for Section 4: • Please only fill in your ‘Employee ID’ and leave the rest of the fields blank (including Program Administrator’s name as this is not referring to your reporting manager). • Complete Sections 3,5 and 6 in full. Section 9 is the be left blank as there are no verifying officers within HP • Read the declaration at section 7, sign and date. • American Express now requires ALL new Corporate Card applicants to provide primary identification documents with Corporate Card application forms to verify the applicant’s name and residential address OR name and date of birth. • Primary identification documents include; a current passport, a current Australian driver’s license or an Australian national identity card (police or armed forces). • When completing new applications, please note you must submit supporting documents which establish your identity in the following way: • Engage an external Certified Signatory*, who must authenticate a copy of the original photo identification document as a true likeness

Corporate Card Application Guide • *Certified Signatories include: • – A registered legal practitioner (solicitor) • – A judge (applicable for overseas applicants) • – A magistrate (applicable for overseas applicants) • – A CEO of a Commonwealth court • – A registrar or deputy registrar of a court • – A Justice of the Peace • – A notary public (applicable for overseas applicants) • – A police officer • – An Australia Post agent/manager/owner • – An Australia Post employee with 2+ years of continuous employment • – An Australian consular officer or an Australian diplomatic officer (applicable for overseas applicants) • – An American Express Employee (applicable for overseas applicants) • – A member of the Institute of Chartered Accountants in Australia, CPA Australia or the National Institute of Accountants with 2 or more years of continuous membership • – A registered officer of a financial institution or financial Company with 2+ years of continuous employment. • Forward completed forms to the APJ Amex DAR (Dedicated Account Rep) at APACChargecard@hp.com • Items to note: • Turn around time for your Corporate Card Application is a maximum of 10 working days upon receipt by American Express; missing or inaccurate information may lead to delays. • If you need your card urgently, tick the box on the top portion of your application form & your application will be given priority handling. A fee of $100 priority handling fee will be billed to your Corporate Card Account. • Application status enquiries can be made to Amex Telephone Service Centre at 1300 362 639 – Amex will only be able to advise status once the application has been received from your Program Administrator.
