ANTECEDENTES • Dosis de CsA por debajo de 3 mg/kg a un año se asociaron a un riesgo elevado de pérdida del injerto (periodo 1985- 1998) (Opelz et al. 2001) • Sin embargo (hipótesis) dosis menores de CsA podrían asociarse actualmente a mejores resultados, debido a: • - mejoras en la biodisponibilidad de la microemulsión • - mayor eficacia de la medicación concomitante (MMF y otros)

  6. OBJETIVOS El presente análisis pretende valorar el efecto de la dosis de ciclosporina microemulsión (CsA-ME) a 1 año sobre la función renal del aloinjerto a 5 años, teniendo en cuenta la influencia de la función renal a 1 año y otros parámetros relevantes.

  7. DISEÑO ESTUDIO Y MÉTODOS • MOST es un estudio observacional, internacional y prospectivo • - Participaron 151 unidades de trasplante renal en 38 países • Incluye 2.889 pacientes con datos completos de seguimiento a 5 años • Inclusión • Pacientes de trasplante renal de novo o en mantenimiento • Con régimen inmunosupresor basado en ciclosporina para microe-mulsión (CsA-ME) bajo condiciones de práctica clínica, en el momento de inclusión en el estudio y durante el seguimiento • Datos prospectivos de 2-5 años complementados con datos retrospec-tivos en los pacientes en mantenimiento Función del injerto evaluada por la tasa de filtración glomerular (TFG) Crockcroft-Gault. Análisis de la covarianza (ANCOVA) para evaluar si los diferentes parámetros afectaban de forma relevante la TFG a 5 años, utilizando la TFG a 1 año como covariable. Se valoró el riesgo relativo (RR) de los principales factores para presentar una TFG < 65 ml/min a los 5 años.

  14. CONCLUSIONES • En comparación con dosis más altas de ciclosporina, una dosis < 3 mg/kg a 1 año se asocia con una mejor conservación de la función renal del aloinjerto a 5 años. • Otros factores significativos son la TFG a 1 año, la edad del donante, y en menor medida, el injerto previo, la incompatibilidad HLA y la función retardada del injerto.
