  3. Overige delen combiformulier • Voorlopig nog manueel • Communicatieformulier tussen DA van NVWA en OA van KDS • Toekomst: digitale verwerking met vastleggen keuringsresultaten • Terugkoppeling resultaten naar bedrijf

  4. Wijzigingen voor exploitant • Voorblad: stallijst met bijlage in te vullen door exploitant voor iedere AM-aanvraag (per keuring) • = verzamelblad van informatie van aanvoerdocumenten (aanvoerlijsten en VKI) • Aan de AM DA te overhandigen voor de AM-keuring

Bedoeling stallijst • Efficiënte en effectieve AM-keuring mogelijk maken. • Eenduidige overdracht AM keuringsgegevens naar PM keuring • Aantal dieren aangeboden per leeftijdcohorte • Onbedwelmd slachten • Controle VKI • Bepalen steekproef TSE en welke dieren • Controle SRM (ruggenmerg verwijdering)

  6. Handtekening exploitant  verantwoordelijkheid • Verplichte controles uitgevoerd • Aantallen • VKI • Overige relevante informatie • AM dierenarts • Verificatie gegevens exploitant • AM keuring

  7. BIJLAGE STALLIJST • Daarnaast gegevens individuele dieren inzichtelijk • Per dier: • Diersoort • Identificatie (I&R/transponder) • Geboortedatum • Land van herkomst • Overligger of niet Ingangsdatum in overleg met KNS
