Pediatric Endocrine Metabolic Clinic • Multidisciplinary team of providers • Pediatric Endocrinologists • Registered Dieticians • Physical Therapists • Family Therapist • Family-based approach • Evaluate and manage obesity related health conditions • Focus on making healthy dietary and activity changes

Pediatric Endocrinologist • Evaluate and treat metabolic conditions associated with overweight and obesity • Insulin resistance, pre-diabetes, type 2 diabetes • Polycystic ovary syndrome • Hypertension and hyperlipidemia • Thyroid disease, Cushing’s disease • Fatty Liver Disease • Work with families on making dietary and activity changes that will improve health • Medication therapy, when indicated • Work closely with other members of our Park Nicollet team to provide comprehensive care • Locations: St. Louis Park, Maple Grove, Burnsville, St. Cloud

Registered Dietician • Assess quality of the family’s diet • Provide a plan for dietary change that is realistic for the whole family • Eating for heart health • Importance of energy balance (calories in vs. calories out) • Guidance with meal planning • Label reading • Food portion review • Regular follow-up to monitor progress and provide additional support and education

Pediatric Physical Therapy • Evaluate strength, balance, coordination, and cardiovascular endurance • Develop an individualized home exercise program • Aerobic exercise • Resistance exercise • Establish skills required for lifelong fitness • 3 locations: Burnsville, Maple Grove, St. Louis Park

Family Therapist • Evaluates for mental health concerns • Depression, anxiety • Eating disorders • Assess self-esteem and body image • Help tackle negative thought patterns • Helps family work together to make lifestyle changes • Assess readiness for change • Determine support and barriers • Develop strategies and tools to stay motivated • Counsels on parenting skills

Camp 5210 • In partnership with One Heartland, Park Nicollet and HealthPartners • For children with type 2 diabetes or at risk for type 2 diabetes • A camp for the entire family to get healthy – all while having lots of fun! • July 6-10, 2012 • Friday-Tuesday • Subsidized cost varies by household income $60 - $250

Camp 5210Recommended Criteria • Patient (child) has Type 2 Diabetes • At risk for Type 2 Diabetes (is overweight with a BMI>85th% and has one of the following): • elevated glucose • elevated insulin level • elevated hemoglobin A1C • Signs of insulin resistance • First degree relative with Type 2 Diabetes • A parent (guardian) must attend, siblings 7 years and older welcome
