General Information: • Contractor: Perk Company, Inc. • Awarded: July 28, 2011 • Signed Contract: August 25, 2011 • Project Bid Price: $3,600,000 • Project Administration: ODOT District 12 • Project Length 1.5 miles • Project starts at Euclid Heights Blvd. • Project ends at Bayreuth Road. • ODOT Project Engineer: Al Yost • ODOT Area Engineer: Patrick McCafferty

  3. Work Schedule Scheduled Start Date: April 2012 Scheduled Completion Date: September 30th 2012 Work will generally be from 7:00 am to 3:30 pm No night work Weekend work only if needed to keep the project on schedule

  4. Work Description • Repair the concrete base and resurface with asphalt from Euclid Hts Blvd to Bayreuth Rd. • All of the existing asphalt will be removed • Extensive concrete slab repairs and replacement from Euclid Hts Blvd to Mayfield Road • Additional concrete base repairs as needed from Mayfield Road to Bayreuth Road • Resurface with approximately 5 ½ “ of asphalt • Eastern most lane from Euclid Heights Blvd to Mayfield Road will be removed. • Road will be 5 lanes wide in this area when work is complete. • New curbs and drainage inlets will be installed

  5. Work Description • Replace damaged portions of curb throughout the project. • Approximately 40 driveway aprons will be replaced (at locations marked for curb replacement) • Upgrade curb ramps to ADA requirements. • Traffic signals at Severance Center Access and Hollister Road will be removed permanently

  6. Phase 1 • Shift all traffic to the East half of the road and make repairs to the West half • Maintain both Northbound and Southbound traffic • From Euclid Hts Blvd to Mayfield Road 1 Southbound lane 2 Northbound lanes • From Mayfield Road to Monticello Blvd 1 Southbound lane 1 Northbound lane 1 Center turn lane • From Monticello Blvd to Bayreuth Road 1 Southbound lane 1 Northbound lane

Phase 1Intersections • Euclid Heights Blvd / Taylor Road • Left turn lane for Northbound traffic • Left turn lane for Southbound traffic • Mayfield Road / Taylor Road • Right turn lane for Northbound traffic • Left turn lane for Southbound traffic • Lanes on Mayfield Road not changed • Monticello Blvd. / Taylor Road • Left turn lane for Northbound traffic

Phase 2 • Shift all traffic to the West half of the road and make repairs to the East half • Maintain both Northbound and Southbound traffic • From Euclid Hts Blvd to Mayfield Road • 1 Northbound lane • 1 Southbound lane • 1 Center turn lane • From Mayfield Road to Bayreuth Road • 1 Northbound lane • 1 Southbound lane
