  3. Log In or First Time User Registration First time users will need to register as a new user with the website and establish an User Name & password

  4. New User Registration You will need to know your Employee Number. *Once you have logged in, you will be directed to the following check status page.

  7. Request link: General Information By clicking on Request under the View section, you will be able to see your Personal Information, at the time of the request. This information will include your demographic information, primary contact information, as well as work information.

Request link: General Information Also, in the view section, you will be able to see the General Information of the leave request, including the leave request number and status, the Initial Request information request, and the Leave Information, which gives the leave reason and whom the leave is for.

  9. Viewing Leave Details By clicking on Details under the View section, you will be able to see the details of the leave request.

  10. Viewing Leave Details The details page will allow you to view the frequency(ies), the decision(s), the date range(s), if certification was due, when it was received, and what leave plans the leave is protected under.

  11. Reporting Using FMLA time You will not be able to report time beyond the end date of the claim.

  12. Reporting Using FMLA time Time will be reported in hours and minutes, for up to 5 days at a time.

  13. Confirming Time Reported You will need to confirm the time report is correct and confirm the time to complete reporting time. If an error is received, the you will have the opportunity to go back and correct the error before moving on. If the error cannot be corrected via the website, the you will need to contact FMLASource directly.

  14. Time Reported Successfully Once the time has been successfully report, you will receive a confirmation page. From this page, the you can return to the check status page.

  19. Short Term Disability ?’s (Continuous Only) Employees who are applying for Short-Term Disability need to complete some additional questions.

  20. Sucessful Claim Submission Once you have completed the request opportunity to print the appropriate medical certification form to take to your doctor. FMLASource will also send the documentation to you.

  21. Letter History The screen will display all the letters that have been sent to you. Here you can verify dates and types of information that was sent.
