2. OBJECTIVES To introduce a project that can reach service providers in the long-term care arena.
Provide suggestions for replicating in rural communities.
3. MISSION STATEMENT The Healthy Aging Coalition of East Texas will promote quality of care by providing educational opportunities and recognition programs for the purpose of enhancing the self-image of direct care staff in long-term care as healthcare professionals.
4. VISION A network of professionals specializing in gerontology dedicated to promoting quality of health care through education, recognition and research.
Each group will develop their own depending on their needs assessment.
5. HOW TO GET STARTED Use area Directories
Assisted Living Facilities
Nursing Homes
Contact the local Area Agency on Aging
Make weekly visits to area facilities to meet with the Administrators/DONs
Plan and Invite all Administrators to a Lunch Meeting of your place of choice
Gather Needs Assessments from all facilities for
CNA/PCAs
Nurses
Administrators

PRESENT MEMBERSHIP The HACET (Healthy Aging Coalition of East Texas) consist of 45 regional long-term care professionals and their Director of Nurses (DONs), Dietitians, Pharmacists, Home Health, Hospice, State, and Assisted Living facilities. Representatives from state agencies also attend and participate

PREPARE Train the Trainer Disaster Preparedness Training (special grant offered by Mather LifeWays Institute on Aging of Evanston, IL)
10. STAR AWARD HACET was organized to promote quality of care by providing educational opportunities and recognition programs for the purpose of enhancing the self-image of direct care staff in long term care.
Each facility participates by submitting a nomination for this award.
11. STAR REQUIREMENTS Requirements for this Nomination:
Good work ethic;
outstanding job performance;
team participation; and
an excellent attitude with residents, family members and other employees.
Nominations will also be accepted for those who are involved in an unusual event or happening that displays an act of kindness and character.
12. AWARD FOR THE STAR Each month the recipient of the Star Award will be
Invited to attend the HACET lunch meeting to receive:
$25.00 gift card, a Certificate of Recognition
Honoree's picture and write-up will be submitted to the local newspaper to further honor the work they do and the organization involved.
The honoree will also be recognized on our website. This will bring positive stories to the media in our community.
