Methoden • PINET, pragmatische cluster-gerandomiseerde trial • 3e jaars aios HAG, Utrecht • Logboeken, 8 dagen, allepersoonlijkepatiëntcontacten • Klinisch dilemma/probleem • Zoekgedrag • Frequentie, tijdsduur, redennietzoeken, methode, moment • Invloedzoekgedrag • Basismetingen Regulier EBM-onderwijs PINET 3e jaars aios HAG UMC Utrecht 1 jaar na afstuderen R Geintegreerd EBM-onderwijs Maart 2011 December 2013

  8. Resultaten • Deelnemers • 76 aios, 7155 patiënten • Bijna 100 patiënten per aios • Klinische vragen • 1476 klinische vragen (21%) • Meestal therapeutisch (50%) of diagnostisch (37%) • Zoekgedrag • 1155 keer zoeken (78%) • Meestal met resultaat (82%) • Duur: gemiddeld 5.4 minuut

  12. Conclusie • Aantal klinische vragen lijkt laag • Bij vragen zoeken aios relatief vaak, meestal in bijzijn patiënt • Resultaat van zoektocht vaak positief • Opleider/collega net zo vaak gebruikt als NHG-standaard

Discussie • Klinische vraag • Onbewust van vraag? • Logboek: invullen kost tijd… • Selectie: alleen ‘vraag’ wanneer ook echt gezocht is, of zelfs alleen wanneer antwoord gevonden is • Redenen om niet te zoeken • Pragmatische oplossing -> (gebrek aan) tijd op 2e plaats • Moment van zoeken • Van deze tijd…? • Nieuwe dokter?
