  1. 90 Day Implementation Steps DO • “We are pleased with the response from our community partners and those new to the department” • Sustain the Improvement • Continue review of the Leadership Team Charter for SHIP Phase two • Move leadership team operations from a “task force” model to steering committee • Review membership needs and original problem statement • Continue to look for ways to use Quality Improvement tools to guide process • It is important to have the right people at the table. We want to remain open to adding new members as the group evolves • There are many considerations for community engagement, such as organizational agendas; local economic and political climates; and “what’s in it for them” • We did attract a variety of people, but still have difficulty connecting with culturally diverse populations • We found value in learning Quality Improvement tools, especially the flow chart and adapted matrix process Test the theory for Improvement In order to address the problem area the team took the following steps: * Held 3 focus groups with geographically diverse populations; each comprised of 9-13 members * Received over 15 leadership team interest forms * Analyzed focus group information * Introduced ‘Letter of Commitment’ requirements * Developed activities & strategies for selecting Statewide Health Improvement Plan interventions Milestones achieved * Received valuable input from geographically diverse participants * Recruited new partners to be part of the team * Recruited five organizations to sign the SHIP Phase I Letter of Commitment statement * Completed and submitted SHIP Phase II proposal with input from the leadership team * Enthusiasm throughout the process was motivating for participants and staff. * Completed a Leadership Team Charter for roles and responsibilities. PLAN Problem Statement ACT • Aim • By June 1, 2009 establish a community chronic disease prevention leadership team for the general population of Washington County in order to address obesity and tobacco health issues. • Goals • 1.To convene (10-12 member) diverse focus groups in each region of the county to assist with the need and design of the collaborative leadership team structure • 2. To invite and convene leadership team members (12 -18 people) for chronic disease prevention in Washington County by June 1, 2009. • Measures • Task Force and Leadership Team membership will reflect the geographic diversity of the county. • Record the short term participation level of Task Force members. • Document the process and record the Plan-Do-Study-Act steps. • Provide a written document to outline the work of the Task Force for determining the Leadership Team structure, coalition goals and member roles and responsibilities. • Record Leadership Team participation and meeting actions on a regular basis to document progress.
