    2. What is the Gold Standards Framework in Care Homes? A national initiative, the Gold Standards Framework is a practical systematic framework of tools and tasks that aims to deliver a ‘gold standard of care’ for all people nearing the end of their lives in care homes.

    3. The National Perspective Originally developed for primary care teams now used in most GP practices in the UK GSF in care Homes programme developed five years ago Now used in 600 -1000 homes Training run by national team to include 4 workshops

    4. What’s involved ? Three stages Preparation stage 3-6 months . Resource pack includes DVD, Sample letters, After death analysis. Training Stage 4 workshops over 9 months with key tasks Accreditation Stage Once training is complete consolidation, ‘embedding’ the learning

    5. Three Aims To improve quality of care for all residents from admission. To improve collaboration with GP’s ,Primary Care and other Specialists. To reduce the number of admissions to hospital in the final stages of life, enabling more people to die with dignity in the home if that is their wish

    6. Key Tasks/Standards–The GSF 7 C’s C1 Communication.......SC register. PHCT meetings,pt info, advanced care planning C2 Coordinator. Key Person C3 Control of symptoms C4 Continuity Out of Hours Handover form/OOH protocol C5 Continued Learning C6 Carer Support Bereavement Support C7 Care in dying phase LCP

    7. Somerset Project Thirty three Care Homes have enrolled across the county 26 Care Homes with nursing 6 residential homes Already a waiting list

    8. Collaboration with others Specialist Palliative Care Teams Community Matrons Community Psychiatric Nurse District Nurses Pharmacists Allied Health Professinals
