Title: Vitreoretinal operation record Leader: A Fok Sponsor : CW Tsang, PP Yip Facilitator : CO Cheng Members: F Luk, S Mohamed, G Lee, D Ng Start Date: 1/4/12 End Date: 15/5/2012 Last Updated: 7/5/2012 ID: BACKGROUND TARGET CONDITION The hand-written part of the vitreoretinal (VR) operation (OT) record is necessary because the current clinical management system and its input method does not allow for the efficient drawing of detailed diagrams required for VR surgery. Redesign a new OT record form to target the root cause. Standardize the way surgeons fill out OT records. CURRENT CONDITION BENEFITS Standardization of OT recordsReduction of time to fill out OT recordsReduction of workload for OT appointment staffImportant information can be found more easily at follow-up Reduction on storage space required to store medical records Time and money are wasted in filing and producing 3 different forms. A lot of time is spent on filling out OT records. Some surgeons do not complete all fields in all forms. Difficult to find important information during follow-up because they are scattered across multiple sheets of paper. IMPLEMENTATION PLAN PROBLEM ANALYSIS FOLLOWUP Why? Multiple redundant sheets need to be filled out Why? Poorly designed forms leaves very little space for retinal diagram Why? Digitalization of OT records make certain parts of the forms obsolete The project will be reviewed after 3 months Efficiency will be monitored subjectively by satisfaction from VR surgeons, ward round doctors, nurses and staff of OT appointment and MRO.
